Clarion Hospital (Clarion, PA), part of Independence Health System, was among 13 organizations in Pennsylvania presented with a 2024 Achievement Award by the Hospital and Healthsystem Association of Pennsylvania (HAP). Clarion was recognized for its implementation of a care coordination program that ensures patients have access to preventive healthcare and supporting community resources after discharge.
