Clarion Hospital shares care coordination success on This Week Health podcast

This care coordination program earned a 2024 Achievement Award for meeting the needs of chronic disease patients.

Clarion Hospital (Clarion, PA), part of Independence Health System, was among 13 organizations in Pennsylvania presented with a 2024 Achievement Award by the Hospital and Healthsystem Association of Pennsylvania (HAP). Clarion was recognized for its implementation of a care coordination program that ensures patients have access to preventive healthcare and supporting community resources after discharge.

Clarion Hospital’s Care Transition Manager Carrie McHenry sat down with This Week Health TownHall moderator and Baptist Health CMIO Brett Oliver, MD, to discuss how Clarion’s award-winning care coordination program has focused on prevention, education, and coordination of care to assist patients experiencing chronic health conditions. 

In the podcast, McHenry explains the practical steps taken to support patients post-discharge, their focus on social determinants of health, and Clarion’s subsequent achievements - including a drastic reduction in readmission rates for COPD and pneumonia. They also talk about the role of the MEDITECH Expanse EHR in tracking patients' needs and outcomes, as well as the importance of community-based resources to help build trust in rural healthcare.
