Make Home Care part of your population health strategy.

As more care is delivered in patients’ homes, your population health strategy will depend on making that data accessible and actionable throughout your organization. With MEDITECH, providers have a fully integrated Home Care solution for a more comprehensive EHR. Clinicians use three components—Home Health, Hospice, and Telehealth—to support patients at every stage of life, in the comfort of home.

Connect all settings for more comprehensive care.

When sharing the same EHR, home care agencies and hospitals are able to work as a team. System-wide integration is at the heart of our Home Care solution, improving care transitions, treating chronic conditions, and avoiding readmissions.

Share information among providers for safer and more efficient patient-centered care.

Home Care staff have access to:

  • Automated patient referrals
  • Up-to-date demographics, medications, and allergies
  • Hospital discharge summary, H&P, CCD, and more
  • Real-time E-signed 485s, Hospice Plans of Care, and orders
  • All patient appointments
  • Notifications of patients’ ED visits

Hospital staff have access to:

  • Updated demographics
  • Home care visit notes, vital signs, medications, and allergies
  • 485s, Hospice Plans of Care, and interim orders flowing to the physician sign queue
  • Telehealth device data
  • Daily and monthly debits and credits within General Ledger

Go the distance with Telehealth.

With Telehealth, providers keep a close watch on their patients, from a distance. Through integration with smart devices from Ideal Life, patient-captured vital signs—blood pressure, heart rate, weight, and blood glucose readings—flow to the EHR and alert staff of potential issues in real time.

Access schedules and aide documentation on the go.

Get on-the-go convenience to view schedules from any smartphone. Clinicians can quickly check their upcoming schedules and view patient details, such as diagnosis, procedure, medications, allergies, phone numbers, and emergency contacts.

Additionally, they can use common mobile device conventions to dial phone numbers and launch maps, to work more efficiently. Home Health aides can also document patient care right from their smartphones.

Bring compassionate care to patients with life-limiting illnesses.

Staff get the support they need—from coordinating complex hospice admissions to documenting care and scheduling IDT meetings—with our responsive Hospice. Its flexibility enables clinicians to customize protocol-driven care plans to reflect the wishes of patients and their families, while ensuring staff follow best practices. In addition, hospice agencies can easily extend their compassion to loved ones with bereavement care plans that help facilitate activities and mailings.

Navigate governmental mandates with ease.

As staff administer care, our task-driven design helps them comply with state and federal regulations by incorporating them directly into the user workflow. Staff are guided through the care cycle to ensure documentation is compliant and completed within patient- and payer-specific timeframes. "MEDITECH’s Home Health solution is task-driven and designed to help clinicians avoid workarounds. Our clinicians are able to follow the correct workflow without information ever falling through the cracks."

St. Mary’s Healthcare System for Children

"The idea of receiving care at home is a powerful motivator. At home, you’re more comfortable, and you aren’t woken up every four hours. Plus, MEDITECH's Patient and Consumer Health Portal makes it easier to be responsible and follow up with providers. Once patients see that—and understand that the portal will ultimately help them to stay out of the hospital—it’s often enough to get them excited about it."

"During our last audit, the surveyor remarked about how outstanding our reports are. Because there is so much meaningful data in the system, we were able to get the information the surveyor requested very quickly, which helped us to get such great survey results."
