Val Verde Regional Medical Center keeps patients engaged at home through Expanse remote patient monitoring

Remote patient monitoring helps Val Verde stay connected with diabetic and hypertensive patients, leading to improved chronic disease management, even during a pandemic.

Over the past year, COVID-19 has taken its toll on healthcare accessibility - especially for elderly and chronically ill patients, who are at the highest risk for severe disease if they become infected in public spaces like doctors’ offices.

To help keep these patients on track while staying safe at home, Val Verde Regional Medical Center (Del Rio, TX) instituted a remote patient monitoring (RPM) program in conjunction with MEDITECH’s Patient Portal. The program provides hypertensive and diabetic patients with blood glucose monitors and scales that can quickly transmit patient vitals to the cloud and Val Verde’s Expanse EHR

Patients have found these devices to be easy to use, and results can be immediately tracked in the MEDITECH patient portal.
