
A Tribute to A. Neil Pappalardo
It is with profound sadness that we announce the passing of MEDITECH Founder and Chairman, A. Neil Pappalardo, on January 27, 2026 at the age of 83. After founding the company on August 4, 1969, Mr. Pappalardo remained a guiding force and driving inspiration until his passing, establishing a legacy that spans over five decades.
Pappalardo is widely credited as the founder of the Electronic Health Record (EHR) industry. His engineering background, which includes co-authoring the first healthcare-specific programming language MUMPS, helped jumpstart the independent healthcare software industry. In doing so, he pioneered a clear vision of the potential this software would have on building a set of integrated application solutions to automate hospital departments, and created the foundation of what would become known as “one patient, one record" EHR integration. Speaking about the initial hurdles, Pappalardo once noted, “The greatest challenge was simply explaining what software was. Potential hospital customers had little or no idea what we were selling and, more importantly, how it could possibly benefit them."
After graduating from the Massachusetts Institute of Technology (MIT) in 1964, Pappalardo began his career as a programmer at Massachusetts General Hospital. It was there that he first observed the complex, error-prone and paper-intensive clinical processes of the 1960s and recognized the profound need for technology to create a safer and more collaborative care environment. This realization led to the founding of MEDITECH, as Pappalardo once explained, “I realized there's probably a market for this, and the only way I would ever get my work into the marketplace was actually to start a company specifically to carry my work forward.”
Under Pappalardo’s leadership, MEDITECH began with a handful of employees operating out of a small office in Cambridge, Massachusetts, and would go on to become one of the largest and first fully integrated EHR vendors in the industry, today supporting over 2,000 customers across 29 countries.

“Neil was a true visionary whose ambitions were not driven by fame or fortune, but by a profound desire to make healthcare better for everyone,” explains MEDITECH Vice Chairman and former President and COO Lawrence Polimeno, Pappalardo’s first employee. “His dedication was to people — his employees, customers, and the patients they serve. He built a company and a culture that will continue to inspire future generations to focus on innovation, value, and compassion for one another. He was also a great friend and inspiration, and I will miss him dearly.”
As the demand for EHRs skyrocketed, Pappalardo generously shared his time, experience, and knowledge with companies entering the market. He was also deeply committed to advancing education and innovation, notably through his continued engagement with MIT and funding the Pappalardo Fellowship Program.
Pappalardo remained true to his mission of making modern healthcare technology affordable and accessible worldwide, reflecting his belief that every community deserves access to advanced technology. These efforts included leveraging MEDITECH’s Laboratory solutions to help combat AIDS in Africa.

MEDITECH’s Board of Directors states, “Neil was a renowned philanthropist, dedicated to advancing education, research, and innovation globally. He was unwavering in his belief that our company must contribute to ensuring all communities have access to advanced, high-quality EHR solutions. He intentionally focused on deserving communities with the greatest need, rather than only targeting the very well-resourced. His dedication profoundly improved the healthcare industry and the lives of many. It has often been said that if you want to understand the heart and soul of a company, you simply have to watch where it goes."
Pappalardo was an ardent believer in perpetual innovation, insisting that technology companies must continually reinvent themselves to thrive. This core philosophy led him to continually rewrite MEDITECH's software from scratch with a user-centric design. He articulated this philosophy, stating, “We now live by the rules of the technological imperative. We must continue redeveloping our products... we must strive to make the human interface easier to comprehend and easier to use."

Even after stepping down as CEO in 2010, Pappalardo remained invested in the success of the company and took a particular interest in the launch of the company’s groundbreaking platform, MEDITECH Expanse, the industry’s first web and cloud-native EHR. Pappalardo continued in an advisory role, helping to ensure Expanse was thoughtfully designed to put the human experience first, continuing his lifelong mission to truly serve those who provide care.
Pappalardo often credited the company's enduring success to the passion, ingenuity, and leadership of his staff. By fostering a culture of mentorship and building leadership up from within, he established a strong line of succession that ensured the company's continued prosperity and values long after his departure as CEO.
MEDITECH’s Board of Directors reflects on Pappalardo’s influence, stating, “With heavy hearts, we extend our deepest gratitude for Neil's vision and dedication. His legacy will forever guide our path forward.”

Pappalardo leaves behind his wife of 61 years, Jane; their four children; 13 grandchildren; and an extended, loving family.
The company announced that Michelle O'Connor, President and CEO, will serve as the Interim chair until the regularly scheduled April board meeting. Neil's family intends to proceed with his preferred plan to maintain MEDITECH's current ownership structure with no anticipated changes to the company's operations or ownership model.

My life and my family would not have been the same without the company and culture that Neil created. I echo all the comments able how humble and kind Neil was. Unlike some other EHR companies, Neil allowed us to create complimentary companies and live and prosper in the MEDITECH ecosystem. I enjoyed a trusted relationship with MEDITECH and appreciated a company that valued integrity and loyalty above all else. My condolences to all of my MEDITECH family & friends and especially to the Pappalardo family. Hard to put into words the impact Neil had on my life and career. Preparing him for his (rare) media interviews was among the most intimidating -- and thrilling -- endeavors of my career, and I remember with crystal clarity just how gracious he was and how much time he was willing to spend (nearly an entire day) 1-1 with a relatively junior member of his management team. I have worked at MEDITECH for 33 years and while I can't say I interacted with Neil often, I witnessed his larger than life personality at MEDITECH picnics, company meetings, and holiday gatherings.I know staff have been touched by his helping hand, always there with a kind and thoughtful gesture.
I saw how much he enjoyed the people around him and how humble he stayed through his accomplishments and accolades which were many.
He was generous, kind, talented, and inspirational - a role model like no other!

A sad day for all MEDITECH'ers , Neil's vision for the future in healthcare was unparalleled, I was fortunate enough to meet him on serveral occasion on my visits to Boston and he always made time for a quick chat, but the highlight was attending a small group training session that Neil presented to a few of us from South Africa on developing binaries. Down-To- Earth Visionary
During my first week at MEDITECH almost 30 years ago, I was completely lost in the Canton facility, frantically searching for the auditorium so I wouldn't be late for the new hire orientation. As I wandered aimlessly, a man dressed casually in a t-shirt on that hot August day noticed me. "You look lost, do you need some help?” he asked. “Yes, please!” I replied, explaining I was trying to find the auditorium. He immediately said, “I know exactly where that is, let me show you.”He walked me right to the door and said, “Welcome to MEDITECH.” I remember thinking, “Wow, what a genuinely nice person—I’m going to like it here!” A few minutes later, as I sat with the other new hires, the company's hierarchical organizational chart was presented. Imagine my surprise when I saw the name and picture of that kind, unassuming man who had just helped this directionally challenged new hire—he was at the very top of the chart.
Neil was an amazing visionary, yet he was just like everyone else—unassuming, never putting on airs, and never giving the ‘do you know who I am?’ vibe. May he rest in peace. My deepest condolences go out to his family.

I joined MEDITECH because of the strong reputation it had for valuing its employees. This focus on staff and their families was a precedent set by Neil. During my 13 years at MEDITECH, I had the pleasure of having some brief interactions with Neil. It was a sad day at MEDITECH when we learned of Neil's passing. He was such a kind and genuine man. It goes without saying that his vision, values, and leadership shaped our company and continue to guide us today. I'll always remember how kind he was to my parents who were visiting from Florida and instead of me skipping the company picnic, he let me invite them to come along. They did not live too far from each other in Florida and he saw us walking so he picked them up in his golf cart and they talked for quite a bit. My parents were so complimentary of what a wonderful person he was and the fun atmosphere of the day. My deepest condolences to his family during this very sad time.Kind Family Man

I have been with MEDITECH for 26+ years, met my wife here and spent most of my adult life working at a truly family friendly company started by Neil. He was truly a visionary in the Health care industry. My wife spent time working 2 desks over from Neil, desks!! , not corner office. She was amazed that the founder of the company worked at a desk just like she did but that just showed how Neil saw himself, as a worker and leader but not above anyone else. He was approachable and generous, my children grew up attending the company picnic and were old enough to interact with him on a couple of occasions and see the commitment and generosity that Neil had to the company and it's people. Legacy well earned
I was saddened to hear of the passing of Neil Pappalardo, a quiet giant in healthcare IT and the visionary founder of MEDITECH.I was a relatively new graduate when I began my career at MEDITECH, coming from an academic laboratory environment. What I found at MEDITECH was something truly special and I was naïve enough to think that all corporate jobs were like that. A workplace that was warm and inviting, designed for collaboration. Interesting artwork on the walls. Elaborate company picnics. Months of training before ever meeting a customer. The opportunity to purchase stock in a growing organization. An executive team with strong female representation, long before that was common.
That culture did not happen by accident. It was the vision of Neil.
Although I only met him in passing, his impact on MEDITECH, and on the EHR industry as a whole, cannot be overstated. He led without ego or fanfare. He didn’t hide behind a corner office. He sat in a cube like everyone else. He was accessible, thoughtful, and deeply committed to giving back, both to his people and to the broader community through his generosity and philanthropy.
Neil Pappalardo built more than a company. He built a culture that shaped careers, set a standard for leadership, and left a lasting imprint on healthcare technology.
I am grateful to have been influenced by his leadership and the values he embedded in MEDITECH. My thoughts are with his family, friends, and the many people whose lives and careers were touched by his work.

I had the good fortune to work for Neil in 1973. He was more than a manager and a leader. He was more like a force of nature. I can't describe everything I learned from him. Besides his technical skills, I learned from his example of how to run a business ethically. He taught me to admit my mistakes and then work to correct them.His loss is a great loss to his family. to Meditech. to his community, and to our country. I am so fortunate to have known Neil.
