KLAS Arch Collaborative industry research shows that documentation burden is among the most commonly reported contributors to nurse burnout, with outdated and inefficient assessment documentation identified as a primary driver. To address this challenge, MEDITECH recommends that organizations complete regular maintenance and optimization of their nursing assessment documentation. One effective approach is to conduct an annual review—akin to “spring cleaning”—to evaluate relevance, efficiency, and clinical value. Although the first iteration of this process can be demanding, establishing a regular cadence will streamline and simplify future efforts.
With this in mind, MEDITECH developed the Mitigate Documentation Burden guide to recommend strategies for undertaking this initiative, including suggestions for routine maintenance, potential enhancements, and policy changes designed to reduce documentation burden while supporting safe, high-quality care.
