MEDITECH’s EHR Steers Avera McKennan ED Nurse Navigator Program to $475,000 Annual Savings


Most emergency departments across the US face a similar problem: caring for patients who frequent the ED for non-emergent issues. To meet this challenge head-on, Avera McKennan & University Medical Center developed an ED nurse navigator program to manage these “super utilizers.”

The program — fully automated in the MEDITECH EHR — assures that these at-risk patients receive the ongoing care and support they need, without draining the ED of valuable resources. As a result, Avera is saving $475,000 annually, as well as decreasing non-emergent ED visits by 13.7 percent.

Avera’s EHR alerts providers that a program participant has returned to the ED, so they can use the patient’s ED care plan to manage his or her care across the enterprise. For example, a nurse navigator and an outpatient clinic coordinator may simultaneously access the patient’s care plan to review the problem list and discuss potential interventions.
