MEDITECH Honors Caregivers During National Home Care and Hospice Month
This November, take the time to thank those nurses, therapists, aides, and social workers who are helping patients to improve the quality of their lives outside hospital walls. During National Home Care and Hospice Month, MEDITECH honors the community of caregivers who are making sure that patients and their families always have the support they need, wherever they need it.
“We should all recognize the value of our dedicated home care and hospice clinicians,” says Cathy Turner, BSN, MBA, RN-BC and associate vice president at MEDITECH. “Because of them, aging and chronically ill patients can receive treatment in the environment where they are most comfortable. Quality care delivery happens wherever the patient is.”
Here’s just a few of the MEDITECH customers who are improving home care quality for their patients:
Sparta Community Hospital (Sparta, IL) is monitoring vital sign data collected in patients’ homes and sharing with providers in real-time, thus helping to improve compliance and reducing unnecessary ED visits.
Fairmont Regional Medical Center Home Health (Fairmont, WV) received high marks this year during an audit by the Joint Commission, as well as a CMS survey.
Hospitals within CharterCARE Health Partners (Providence, RI) recently racked up more than a dozen quality awards, accreditations, and certifications.
