Long Term Care - Post-Acute Services
Unite care settings for safer transitions.
One patient. One record. One statement. That’s how our Long Term Care features help your organization focus on patients and residents. Integrated and interoperable, our solution unites all settings, so you can deliver the highest quality care, meet regulatory requirements, and optimize reimbursement.
Our solution enables long-term acute care hospitals, rehabilitation hospitals, skilled nursing facilities, and outpatient clinics to:
- Focus on the Patient
- Streamline Resident Assessments
- Report Outcomes
- Manage Resident Trust Accounts
- Maintain a Central Business Office
Focus on the Patient
Clinicians are able to create interdisciplinary plans of care and document care delivery with our Post-Acute Care toolset. Workflows specifically designed for your care settings are equipped with decision support for safety and efficiency. From a central location, providers can enter and manage unlimited orders, which can then be used to coordinate and drive care.
View real-time patient and resident information captured anywhere in the continuum, and share data with other networks via a Continuity of Care Document exchange suite.
Streamline Resident Assessments
MEDITECH’s embedded Resident Assessment Instrument (RAI) drives your care plans in any setting, including swing bed locations. As you complete the RAI, this comprehensive, fully compliant assessment triggers corresponding care area assessments (CAAs) and allows clinicians to add appropriate problems to the plan of care. Our solution also calculates activities of daily living (ADL) scores, determines Resource Utilization Group (RUG) categories, and shares all relevant Minimum Data Set (MDS) data with Patient Accounts.
Keep your organization’s quality initiatives on track — monitor performance and outcome measurements (NASMHPD), report Peer Review Organization (PRO) data, support state and federal reporting requirements, and compile census reports with level of care detail.
Manage Resident Trust Accounts
Use our Resident Trust Accounting tool to manage your residents’ financial transactions: depositing Social Security benefits and cash, withdrawing funds for beauty/barber services, and generating statements and reports.
Maintain a Central Business Office
Centralized and decentralized billing, conflict checking, denial management, and integrated charge capture all help to reduce AR days and optimize your reimbursement. Our toolset helps you to meet federal, state, and intermediary billing requirements, and also supports electronic submissions to Medicare and Medicaid.
